Untuk bergabung menjadi distributor 4life, anda harus menyiapkan data-data sbb:

1. Mengisi Formulir Pendaftaran
   
2. Fotokopi KTP
3. Fotokopi NPWP *
4. Fotokopi Kartu Keluarga (KK) *
5. Fotokopi Buku Tabungan
6. Membeli Paket Enrollment Distributor Baru
